Step 1: Application
Upon contact, parents receive an application package relevant to the child's age, and consisting of a questionnaire and forms to gather background and diagnostic information. Parents should complete the questionnaire and attached forms and return them to the LRC in order to be given an appointment.
Step 2: Initial Intake Interview
Applications are presented to the Director of Professional Services (DPS) and assigned to a LRC specialist. The LRC Specialist contacts the parents to schedule an initial intake interview. With the parents' help and information provided, the LRC specialist determines the specific areas in which the child is experiencing difficulties. The DPS and the LRC specialist discuss the information from the initial interview and refer the child for
(i) Assessments
(ii) Medical &/or Para- Medical consultation, or
(iii) Specialized Services. Parents receive a written report with the Center's recommendations.
Step 3: Assessments
The LRC specialist(s), in agreement with the parents/guardians, schedule an assessment for the child, after which, the child may be referred either
(i) to further assessment(s),
(ii) to services, or
(iii) for discharge, according to the specialist(s) findings. The Assessment is followed by a Parent Conference Meeting, and later parents are given a Written Report including findings and recommendations.
Step 4: Specialized Services/Consultations
If it is decided and agreed between the parents and the specialists, the child may be referred to specialized services or Consultation in one or more areas at the LRC. |
